Abstract
The problem in this research is a cultural organization problems in hospital and community health centers have not positive impact yet to the officers in giving health service in Kepulauan Anambas Regency. The study of organization culture using the theory from Leebov and Scott (1994) especially in the aspect accountability. Qualitative research methodology is chosen to answer the problems of formulation and meaning interpretation with organization culture approach. The data obtained from the interview, observation, and documents. In determining the informant, the researcher do it purposively based on the needs and research purposes. Thus, the informants divided into two categories, namely health officers and society. The research finding shows that the organization culture of hospitals and Public Health Centers has not been suitable with Leebov and Scott’s theory. It can be seen from the officers that only work under instruction and habit. They do not have job description that accommodate the patients’ need; the officers run the Standard Operating Procedure that is improvised that cannot satisfy the needs of the patients. The lack of support in implementing accountability policies, namely officers have not been given training to deal with problems that arise. Lack of support also causes problems to not be resolved properly. Thus, the policy about SOP, the incentive of health workforce, and competence development for the entire health workforce, is the fundamental for the organization culture improvements to make good quality of health service.

Abstrak
Permasalahan dalam penelitian ini adalah budaya organisasi pada rumah sakit dan puskesmas belum berdampak positif terhadap petugas dalam pemberian layanan kesehatan yang berkualitas sesuai tuntutan kebutuhan masyarakat di Kabupaten Kepulauan Anambas. Kajian budaya organisasi menggunakan teori Leebov and Scott (1994), namun dalam penelitian ini fokus kepada aspek akuntabilitas. Metode penelitian kualitatif dipilih untuk pengungkapan proses dan interpretasi makna dengan pendekatan budaya organisasi, sehingga dapat menjelaskan data yang diperoleh dari wawancara, observasi, dan dokumen yang berhubungan dengan penelitian. Penentuan informan dilakukan secara purpossive, sesuai dengan keperluan dan tujuan penelitian, informan dibagi menjadi dua kelompok, yaitu petugas kesehatan dan masyarakat. Temuan penelitian menunjukkan, budaya organisasi pada rumah sakit dan puskesmas belum sesuai dengan teori Leebov and Scott, dapat dilihat dari: petugas melayani pasien sebatas perintah dan kebiasaan, belum memiliki uraian tugas yang mengakomodir kebutuhan pasien; petugas menjalankan Standar Operasional Prosedur (SOP) yang dibuat seadanya, belum dapat memenuhi kebutuhan pasien; minimnya dukungan dalam penerapan kebijakan akuntabilitas, yaitu petugas belum diberi pelatihan untuk menangani persoalan yang muncul. Kurangnya dukungan juga menyebabkan persoalan tidak dapat diselesaikan dengan baik. Kebijakan tentang penerapan SOP, pengembangan kompetensi bagi seluruh tenaga kesehatan, menjadi dasar bagi perbaikan budaya organisasi untuk menghadirkan pelayanan kesehatan yang berkualitas.
