Analisis Kualitas Layanan Kesehatan untuk Narapidana dengan Diabetes Mellitus di Lapas Kelas IIA Kendari

Latar belakang penelitian ini berangkat dari pentingnya pemenuhan hak atas pelayanan kesehatan yang layak bagi seluruh warga negara, termasuk narapidana yang berada di lingkungan pemasyarakatan. Di Lapas Kelas IIA Kendari, meskipun jumlah kasus diabetes tidak dominan, data kematian menunjukkan bahwa komplikasi penyakit ini menjadi salah satu penyebab utama. Penelitian ini bertujuan untuk menilai kualitas pelayanan kesehatan bagi narapidana pengidap diabetes mellitus serta mengidentifikasi kendala dalam pelaksanaannya. Penelitian menggunakan metode kualitatif deskriptif dengan teknik pengumpulan data melalui wawancara mendalam, observasi langsung, dan studi dokumentasi terhadap narapidana pengidap diabetes serta tenaga medis klinik lapas. Hasil penelitian menunjukkan bahwa kualitas pelayanan kesehatan secara keseluruhan berada pada tingkat cukup baik Dimensi aksesibilitas dan hubungan antar manusia menonjol sebagai yang terbaik berkat lokasi klinik yang strategis, layanan gratis, ketersediaan darurat 24 jam, serta interaksi tenaga medis yang ramah, empatik, dan non-diskriminatif yang membangun kepercayaan serta kepatuhan pasien. Dimensi keamanan, kenyamanan, informasi, dan ketepatan waktu dinilai baik karena penerapan protokol isolasi yang ketat, lingkungan klinik yang bersih dan inklusif, penyampaian edukasi yang jelas, serta respons cepat terhadap situasi darurat. Namun demikian, dimensi kompetensi teknis, efektivitas, efisiensi, dan kesinambungan masih tergolong kurang optimal. Hal ini disebabkan oleh minimnya pelatihan berkelanjutan bagi tenaga medis, kegagalan dalam pencegahan komplikasi fatal, ketergantungan pada rujukan eksternal akibat keterbatasan sumber daya, serta hambatan administratif dalam proses rujukan elektif yang cenderung lambat. Kendala utama yang ditemukan mencakup keterbatasan infrastruktur medis seperti absennya laboratorium dan stok alat diagnostik, minimnya anggaran operasional, serta prosedur birokrasi yang memperlambat akses terhadap layanan spesialis di luar lapas.
